Abstract

Background: Ariseinmultidrug-resistanttuberculosis(MDR-TB),whichisdefinedastheresistancetothetwomosteffectivefirstlinetherapeuticdrugs,Isoniazid(INH)andRifampin(RIF),threatensglobalpublichealthworldwide. Resistanceof Mycobacterium tuberculosistoINHresultsfrommutationsinseveralgenesmostcommonlyinkatGgene,andresistancetoRIFisduetomutations in rpoB gene. Therefore, rapid diagnosis of MDR-TB is of high importance in controlling the disease progress and outcome. The accuratedetectionof theresistantTBstrainscanbeacceleratedbydevelopingmoleculartests. Objectives: The aim of the present local study was to isolate MDR-TB from the patients who were the residents in the west of Iran andexaminationthefrequencyof MDR-TBbetweenpatientsof Lorestanprovinceforthefirsttimeandassessthemutationsinthe regionsrelatedtoRIF/INHresistanceviaPCRandsequencingmethods. Methods: In this study, 106 isolates of M. tuberculosis were selected in health centers of Lorestan, Iran from 2014 to 2017. After culturingM.tuberculosisisolatesonLöwenstein-Jensenmedium,classicalsusceptibilitytestingproportionalmethodagainstINHand RIFwasperformed. AfterDNAextraction,PCRandsequencingwereusedtodetectmutationsrelatedtoRIFandINHresistance. Results: Theresultsdemonstrated3.8%,0.9%,and0.9% frequencyforINH+RIF,INHandRIFresistance,respectively. Importantly,4 outof 6isolatesharboredmutationsincodon290of katGgene. Also,theseisolatescontainedmutationsincodon339of rpoBgene. NomutationwasobservedininhAgeneof M.tuberculosisisolates. Conclusions:Theresultssuggestthatmoleculartechniquescanbeusedasarapidmethodfortheidentificationofdrugresistance inclinicalisolatesof M.tuberculosis. DNAsequencinghasahighsensitivityforthedetectionof resistancemutationstoRIFandINH inMDR-TBcases.Also,theresultsshowedthatthemostfrequentresistanceassociated-mutationsoccurredincodon290ofkatGand codon339rpoBgenesegments.
